Cleveland Clinic generated $18 billion in operating revenue and completed nearly 16 million patient encounters in 2025, President and CEO Tom Mihaljevic, MD, said during a Jan. 26 annual address.
The academic medical center also reported an operating margin exceeding 4%, an increase from the nearly $16 billion in revenue and a 1.7% margin in 2024.
Dr. Mihaljevic’s remarks focused on expanding access and supporting the workforce, which now includes 83,000 caregivers worldwide. Key initiatives include a new cancer center and medical office building set to open in the second quarter of 2026 at Fairview Hospital, the launch of fundraising for a new hospital in West Palm Beach, Fla., and a planned expansion of the Richard E. Jacobs Campus in Avon, Ohio.
Looking ahead to 2026, Dr. Mihaljevic emphasized a continued focus on safety, quality and patient experience. Priorities include documentation, harm prevention and trust-building. He noted that caregivers too often face workplace violence.
“Cleveland Clinic has treated this crisis with urgency, and caregivers no longer accept violence as part of working in healthcare,” Dr. Mihaljevic said. “We invested in security and training to protect us. Reporting is up, escalation is being caught sooner, and injuries are declining.”
He also highlighted the potential of emerging technologies, including artificial intelligence, to improve affordability and access. AI will help address key challenges by integrating staffing, scheduling, supply chain and patient care in real time, predicting clinical risks, personalizing care and automating administrative tasks, he said.
“As the delivery of care evolves, so will our roles,” Dr. Mihaljevic said. “What is certain is that Cleveland Clinic will embrace technology to remain a leader in healthcare. … While we stay true to our mission, Cleveland Clinic’s values evolve with the changes in healthcare. By seeking (caregivers’) input, we have refreshed our values to be timeless, contemporary and reflective of all caregivers. I am proud to share them today: Serve with heart. Succeed as one team. Shape the future.”
He also outlined several community-focused initiatives, including the opening of new food pantries in Northeast Ohio, support for lead hazard removal in homes and childcare centers, investment in renovations at the Hitchcock Center for Women — a facility focused on recovery and family stability — and $1.49 billion in commitments to research, education and community programs.
