More than 150 healthcare organizations have been accepted to participate in the initial rollout of CMS’ ACCESS Model.
Five things to know:
1. CMS has also extended the application deadline for the model to May 15 ahead of its July 5 launch, according to an April 13 news release.
2. Applications for the ACCESS Model — short for Advancing Chronic Care with Effective Scalable Solutions — began Jan. 12.
3. The organizations accepted include technology-enabled care companies, virtual care startups and a handful of community health centers and hospital-affiliated groups. The health systems accepted include Martin Luther King Jr. Community Medical Foundation, part of Los Angeles-based MLK Community Healthcare; Medical University Hospital Authority, the clinical enterprise of Charleston, S.C.-based MUSC Health; and New York City-based One Brooklyn Health.
4. Most of the organizations accepted have not previously served Medicare beneficiaries and are expected to expand technology-supported care options for chronic conditions such as high blood pressure, diabetes, chronic pain and depression.
5. The voluntary ACCESS Model targets conditions affecting more than two-thirds of Medicare beneficiaries. Participating organizations must meet requirements including Medicare Part B enrollment as providers or suppliers, as well as compliance with licensure, data privacy and security, outcome reporting and other quality standards.
